Q. What information is required to schedule an appointment?

A. The information listed below is required to book an appointment.

-Name, DOB, Health Card Number, Phone Number and Weight
-Name and Location of Referring Physician that signed the requisition
-Name of Exam that is marked off on the requisition.

Q. Do I need to arrive early for my appointment?

A. Please arrive 10 minutes before your appointment so we can complete the registration process.

 

Q. What is required at the time of registration?

A. Requisition that is given to you by your referring physician and Health Card number or any other insurance forms covering the cost of the exam must be given to the receptionist at the time of registration. If no health coverage is available, our Central location will accept Cash, Debit, Master Card and Visa. All other locations are cash transactions only.

 

Q. What is the procedure on picking up my images for a specialist appt.?

A. Please call 519-672-5270 to request these images. Information required is the patient's name, DOB, Health Card number, exam and day it was done as well as the specialist's name. LXA requires 48 hr notice to complete this request. When arriving to pick up your CD, you must provide picture ID as well as signing a consent form.
If you are picking up a CD for someone else, a consent form must be completed by the patient and provided to LXA. This can be returned to us the same day you arrive to pick up the CD. Photo ID must be provided by the person picking up the CD. Blank consent forms can be emailed to the patient per their request to be completed.

We now have the ability to provide our patients with access to their medical imaging records through our partership with PocketHealth. Once you have access to your medical images, you can choose to share a copy via email, burn your own CD or even copy your files to a USB. Online enrollment is very efficient and you will not have to come into our Clinic to submit paperwork or to pick up a CD.

Q. How long is my requisition valid for?

A. Requisitions are only valid for 3 months from the time they are issued from the referring physician.


Q. Do you allow use of recording devices at LXA?

A. LXA does not permit the use of cell phones, cameras or any recording devices in the exam rooms.
